During their stay at Heatherwood Court a service user will be allocated with their own primary care team. With the service user’s help the care team draws up an individualised plan of care to enable the service user to be actively involved in their own recovery action plan which is designed to help them get better.
Service users work with us to identify the goals and values that are important for them and to gain greater understanding of the behaviours that they may have struggled with, and which have led to them being admitted to hospital.
Once the Recovery Action Plan is completed the Heatherwood team creates a programme of care and treatment aimed at helping a service user move on from hospital and build their life up again in the way that they want.
Care and treatment plans may involve psychological treatment and support and, where appropriate, medication planning.
